Participating in the survey is easy. You can either:

• complete the online survey, 
• download a survey, and e-mail or post it back to us
• request a postage prepaid survey by contacting us (to return to us by post).  

Students under 18 years of age are welcome to participate in this survey. To participate, please complete and return (by mail or fax) the student/guardian consent form, available here.

Hints: for the online surveys, use the navigation button at the bottom of the survey to go to the next page.   If you return a survey by e-mail to us, don’t forget to save your completed surveys first.
